COVID Vaccination Info

Unvaccinated students will not be permitted on campus after Jan. 10, 2022

To remain at Poly, students must have their first dose no later than Nov. 21. Your second dose must be completed by Dec. 19.

Beginning Monday, January 10, 2022, eligible students who do not have proof of vaccination against COVID-19 will not be permitted on school campuses and will be referred to the District's independent study program at City of Angels, unless an exemption or conditional admissions apply.

Where can my child get vaccinated?

At an LAUSD site. Click here for dates and locations in Local District Northeast. You can schedule an appointment in DailyPass, or walk-up.
At most drug stores with an appointment. (Click here to make an appointment at Rite Aid, CVS, or Walgreens.

To get a vaccination at an LAUSD site, students must be at least 12 years old. Parents must consent to the vaccine. Students can bring consent form to the appointment.

How do I show proof of vaccination if I get vaccinated outside LAUSD?

If a student has been vaccinated, but not via a mobile clinic, they must upload a photo of their vaccine card onto Daily Pass. A step-by-step guide is here.
