Form (attached LINK) Must be Filled out by Employers, Student and Parent. Form may also be picked up in the Work Permit Office -

Poly's Work Permit Office is located in Room 95.

Completed Form should be turned for approval.  (Pending applicant meets qualifications)

All complete forms may take UP TO 2 school days to process.

WORK PERMIT MAY BE REVOKED (CANCELED) IF YOU NO LONGER MEET THE REQUIREMENTS - GPA DECLINES OR ABSENCES INCREASE.
