Mobile Clinic for Sports Physicals

If your Student Athlete has not received a sport physical, they can take care of it on September 4, 2026. The CCHC Mobile Clinic will be on site to conduct physicals that day. Please make sure your student completes the consent form (linked below) and turns it in to their coach or to Ms. Dougall, Athletic Director before August 28, 2025.
