Attendance Policy

ATTENDANCE POLICIES AND PROCEDURES

 

One of the most important factors of success in school is

good attendance and punctuality.

 

RETURNING FROM AN ABSENCE

When you are absent from school, you must bring a note to the Attendance Office between 7:00 a.m. and 7:45 a.m.  The note must include your name, birth-date, date(s)   of absence, reason for absence, and parent/guardian’s signature.

ABSENCES

    • Inform the Attendance Office if your child is going to be absent. Students must always have a note, which includes the date of the absence, the reason and a parent’s signature.
  • Being truant from school means being absent from class without permission from parents or school personnel.  Please be aware that continued truancy will lead to additional consequences.
  • Telephone calls will be made to inform parents of their child’s absence
  • Parents/guardians are encouraged to visit the Attendance Office periodically to review their child’s attendance record.

 

EARLY DEPARTURES

  • Student must bring a note signed by a parent to the Attendance Office before school. All notes will be verified. Any student wishing to leave campus early without a note must have a parent/guardian present (with proper identification) to check them out through the Attendance Office. Anyone who leaves campus without properly checking out through the Attendance Office will be considered truant.
  • A student can be released and information about a student may be given ONLY to those persons on the student’s school records.

 

CHANGE OF ADDRESS & TELEPHONE NUMBERS

  • Emergency cards listing current telephone numbers and addresses must be turned in to the the corresponding SLC office at the beginning of the school year. It is the responsibility of parents/guardians to inform the school of any address or telephone number changes.
  • Failure to update the address of residence may prohibit the student from qualifying for a continuing enrollment permit at Poly HS.

TRANSFERRING STUDENTS

In order to withdraw your child from Poly HS and transfer your child to a new school, come to the Attendance Office with your current identification card and complete the “Parent Assurance Letter”.
